History

Beginning in the 17th century, the concept of poor houses (also called almshouses) was brought to America by British settlers. All orphans, mentally ill and poor elderly are placed into this shared environment. These poor homes provide a place where they can be given shelter and daily food. Poor homes continued to exist until the early 20th century despite criticism they faced. Most of the criticism comes from the condition of poor homes. The Great Depression struck poor homes because there were many people in need of help and care but not enough space and funds in poor homes. Because of Muck Raking in the 1930s, the unfavorable living conditions of poor homes were exposed to the public. Poor homes are then replaced by various types of dwellings for the elderly. This new residence is called a nursing home or a nursing home or also known as a recovery home. These board-and-care houses will provide basic level meals and treatments in a private setting for a fee. The nursing and care homes proved successful and ahead of World War II, a new way of nursing homes began to take shape. As time passed, the government identified the problem of people spending a lot of time in the hospital. To combat these long periods in short-term arrangements, care homes and careers are beginning to turn into something more public and permanently federally funded and federally funded. From here, in 1965 the nursing home was a solid fixture. The nursing home is a permanent residence where parents and disabled people (poor elderly and special disabled) can receive necessary medical care and receive daily food. Although nursing homes were initially imperfect, they were a big step above flats and poor homes in law and hygiene. From the 1950s to the 1970s the dynamics of the nursing home began to change significantly. Medicare and Medicaid began to raise a lot of money that would filter homes and the 1965 amendment laws forced nursing homes to comply with safety codes and required that registered nurses were always available at all times. Then in 1987, the Nursing Reform Act was introduced to begin defining different types of nursing home services and then adding the Bill of Rights Citizens. Today the nursing home is very different across the board. Some nursing homes still resemble hospitals while others are more like houses. Residents of a nursing home can pay their maintenance fees, others may receive medicare for a short time and some may use long-term insurance plans. Across the spectrum, most nursing homes will receive medicaid as a source of payment.

Staff

In most jurisdictions, nursing homes must provide sufficient staff for adequate care for residents. In the US, for example, nursing homes must have at least one registered RN nurse available at least 8 hours a day for a week, and at least one licensed practice nurse on duty 24 hours a day.

Nurse

Nursing homes require RN to attend to assess citizens and monitor results. RN's job assignments include applying an accurate maintenance, medication, record keeping and reporting plan for each resident, monitoring and recording medical changes and providing direction to licensed nursing assistants or licensed nurses or licensed nurses. In order to be allowed to work as a Registered Nurse, one must complete a nursing program at one of several levels of education, including associate and undergraduate programs, a hospital-based diploma program and a master's degree program. A licensed nurse practitioner completes a one-year approved program. Both registered nurses and licensed nursing practitioners must pass a national licensing exam before they can begin work. The LPN or LVN monitors the welfare of citizens and administers treatments and medicines, such as treating wounds and dispensing prescribed medications as directed by Registered Nurses.

Nurse assistant

The nursing assistant provides basic care to the patient while working directly under LPN/LVN or RN. These basic care activities, also referred to as daily life activities, may include assisting by bathing and accompanying residents, assisting people with food, either serving them or by feeding, moving to and from a bed or wheelchair, making and cleaning a place sleeping, helping with toileting, and answering lamp calls. Training to become a nursing assistant is offered in several high schools, as well as in vocational-technical schools, community colleges, and nursing homes. These programs are usually three months to a year depending on the program. The nursing assistant is then registered in the state health registry. The nursing assistant's degree may range from facility to facility, job titles including Certified Nursing Assistants (CNA's), nursing aides, caregivers, peer care patients, patient care technicians and nursing assistants

Administration

Depending on the size of the nursing home, a nursing home may have a nursing home administrator or executive director. Some nursing homes may have both but their job duties are similar and may include overseeing staff, supplying medical supplies and financial problems. The nursing home administrator/career executive director requires at least a bachelor's degree and some advanced positions may require a master's degree. There are certain classes that are normally taken on this course of study, these classes include the practice of nursing home administration, aging and long-term care, gerontology and aging, and health behavior. You can also find employees of human resources in nursing homes. This work requires at least a bachelor's degree and is usually preferably related work experience. This employee is responsible for all aspects of hiring new employees. Human resource job duties vary but may also include payroll coordination, arranging orientation programs for new employees, interviews, disciplinary action, and ensuring compliance with federal and state laws. Nursing homes are usually licensed and highly regulated under the governing law. Compliance with federal and state legislatures is periodically reviewed for compliance with strict codes of building codes, care plans, behavior and contention among residents, nutrition and diet services, medical services, personal care and care, religious and spiritual practices, pets and recreation program.

Household

The housekeeper is an important part of maintaining a nursing home. Housekeepers play a major role in ensuring that nursing homes are clean and free of disease-causing agents. The housekeeper has a long list of tasks that include floor cleaning, changing linens, cleaning the bathroom, changing towels, washing clothes, emptying the trash, cleaning the room, replenishing supplies, cleaning and polishing furniture, vacuuming and keeping windows and woods clean. These tasks may vary from facility to facility but overall will include basic cleaning. Households do not require a license or a school, but some household chores may prefer previous work experience.

Recreational staff

Recreational staff usually includes an activity director and possibly an assistant of activities depending on the size of the nursing home. The activity of the activity director requires an associate's degree or a bachelor's degree and allows for further certification through the National Council for Therapeutic Recreation. Activities are planned that aim to meet every inhabitant of emotional, intellectual, physical, social, spiritual, and vocational needs. The transition from being self-reliant to being dependent on others and being away from home is often very difficult, which is why activities are important to combat depression and anxiety. Some of the different activities that can be offered include birthday parties, holiday celebrations, book clubs, music events, outdoor activities, discussions and social groups, sports, arts and crafts, pet therapy, religious services and community events. Volunteer involvement is also an important part of nursing home activities considering that volunteers can act as a link between nursing homes and outside communities.

Therapy

Occupational therapy

One of the many services offered in nursing homes is occupational therapy. Occupational therapy may be required after injury or illness to regain skills and to receive support during physical or cognitive changes. Occupational therapy will focus on daily life activities such as bathing, dressing, and dressing up. Occupational therapy also assists with the daily instrumental activities of daily living including home and financial management, rest and sleep, education, work, play, leisure, and social participation. Occupational therapists work to enable the person to safely and comfortably reintegrate into the community by practicing public meals, transferring to various surfaces (chairs, beds, sofas etc.), and will assess the need for home modifications or safety equipment to ensure proper and transition safe. When cognitive and/or perceptive deficits are presented, the therapist will work with the person by teaching strategies to maximize memory, sequence and long attention spans.

Physical therapy

Another important service found in nursing homes is physical therapy. Physical therapy may be necessary after injury, illness or surgery. Physical therapy works with people to help them regain strength, endurance, flexibility, balance, and range of motion. Physical therapy is also used as a way to prevent injuries and accidents by focusing on restoring mobility, improving fitness levels, reducing pain and reaching a certain point of independence overall. There are many conditions that can benefit from receiving physical therapy in nursing home, these conditions include arthritis, cancer-related pain, dementia, Alzheimer's, stroke and incontinence.

Speech language therapy

Speech-language pathology is another service found in nursing homes. Speech language pathologists specialize in working with those who have difficulty with language and/or speech, usually after an underlying injury or diagnosis. SLP will evaluate people's speech. If the person is having trouble with speech, it points to the problem by coordinating the movements and muscles used to produce speech. While problems with language point to people who have difficulty in understanding what they hear and see. SLP will also see difficulty with swallowing food and will evaluate the person to know which part of the swallowing process is not working. Some of the many speech disorders handled by SLP are; Phonology means speech patterns used, Apraxia which means difficulties with the coordination of movement necessary to make sound, Receptive language which means difficulty understanding language, fluency meaning stutter, Expressive Language which means difficulty using language and many other disturbances.

Canada

Long term care facilities are under three main types: private, nonprofit/charitable, and municipal. Regardless of their ownership, the funding aspect, the acceptance criteria, and the cost to individuals are all governed by their respective provincial governments. Due to publicly funded medical care in Canada, all long-term care facilities receive funding from the provincial government for the components of residential health care - nurses and personal support workers. Residents pay daily rates for 'rooms and meals' (accommodation and meals) determined by the type of room chosen, whether shared or private. The provincial government manages waiting lists for long-term care facilities. People who can not afford the monthly fees receive subsidies, and no one is denied for not being able to pay.

United Kingdom

In the UK, nursing homes and nursing homes are organized by organizations in England, Scotland, Wales and Northern Ireland. To enter a nursing home, a prospective patient needs an assessment of their needs and financial condition from their local council. Candidates may also have assessments by the nurse, if the patient needs treatment. The cost of home care means being tested in the UK.

The home care for adults in the UK is governed by the Quality Care Commission, which replaces the Commission for the Inspection of Social Care, and each nursing home is checked at least every three years. In Wales, the Nursing Standards Inspectorate for Wales has responsibility for supervision, in Scottish Social Care and Scottish Social Improvement or otherwise known as the Inspectorate of Nursing, and in Northern Ireland, the Regulation and Quality Improvement Authority of Northern Ireland.

In 2002, nursing homes were known as nursing homes, and residential homes were known as nursing homes.

In April 2009 in the UK, the lower limit of capital was Ã, £ 13,500. At this rate, all revenues from pensions, savings, benefits and other sources, except "personal expenses" (currently £ 21.90), are used to cover the cost of home care. The local council pays the remainder of the contribution if the occupied space is no more expensive than the normal rate of local councils. The NHS has full responsibility for funding all placements if the resident is in an orphanage with nursing that meets the criteria for NHS to continue with Health Care. This is identified by a multidisciplinary assessment process.

In May 2010, the Coalition Government announced the creation of an independent commission on long-term funding of care, which would be reported within 12 months of financing care for the aging population. It submitted its recommendation on Monday, July 4, 2011. The Quality Commission of Care itself has implemented a re-registration process, completed in October 2010, which will result in a new form of regulation outlined in April 2011.

United States

In the United States, there are three main types of nursing facilities (NFs).

Medium care facilities (ICF) are health care facilities for individuals with disabilities, elderly, or no acute illness, usually providing less intensive care than those offered in hospitals or skilled care facilities. Usually ICF is paid personally by individuals or by individual families. Individual personal health insurance and/or third party services such as hospice establishments may recover costs. Board and Care Homes are special facilities designed to provide those who need good residential living and care services. Often referred to as home care homes, this facility can be located in small residential homes or large modern facilities. In fact, most councils and nursing homes are designed for less than 6 people. Councils and nursing homes are usually managed by licensed professionals, including nurses, doctors and other medical professionals. These facilities are strictly regulated to ensure that the best care is provided to the citizens. Councils and nursing homes offer 24-hour assistance to citizens, making them a very popular choice for those who need regular help.

Auxiliary living residence or assisted living facility (ALF) is a housing facility for persons with disabilities. This facility provides supervision or assistance with daily life activities (ADL); ALFs are an elderly care alternative to sustainability care for people, for whom self-sufficient living is inappropriate but does not require the 24-hour medical care provided by nursing homes and too young to live in nursing homes. Living with help is a philosophy of care and service that fosters independence and dignity.

The skilled care facility (SNF) is a nursing home certified to participate in, and replaced by Medicare. Medicare is a federal program especially for the elderly (65) who contribute to Social Security and Medicare when they are hired. Medicaid is a federal program that is implemented with each state to provide health care and related services to those who are below the poverty line. Each country defines poverty and, therefore, meets Medicaid requirements. Those eligible for Medicaid may be low-income parents, children, including the State Children's Health Insurance Program (SCHIPs) and maternal-child health and food programs. seniors, and people with disabilities.

The Medicare and Medicaid Service Center is a component of the US Department of Health and Human Services (DHHS) that oversees Medicare and Medicaid. Most Medicare and Medicaid money is used annually to cover care and home care services for the elderly and the disabled. State governments oversee nursing home permits. In addition, countries have contracts with CMS to monitor nursing homes who wish to be eligible to provide care to Medicare and Medicaid beneficiaries. Congress sets minimum requirements for nursing homes wishing to provide services under Medicare and Medicaid. This requirement is broadly outlined in the Social Security Act, which also entrusts the Secretary of Health and Human Services with the responsibility to monitor and enforce these requirements. CMS is also charged with the responsibility for drawing up legal details and how it will be implemented, which is done by writing rules and manuals.
